INDIVIDUAL STUDENT ATTESTATIONS

We must collect from EVERY family a health attestation showing you understand the elements we are all watching for each day before children are sent to school. It takes one minute to read and one minute to complete and submit the form acknowledging that you have read this over. Even if you are not sure whether you did, go ahead and submit again and we will sort through the info. Thank you for taking two minutes  to complete this. This form only needs to be done quarterly and only for in-person students.
